Effective Monday, July 25, 2011 the attached Critical Incident Reporting form and fax cover sheet will be utilized to facilitate AAU reporting of ADvantage member health and safety issues to the AAU.
Upon discovery of a listed critical incident, the form will be completed by the reporting agency, reviewed and signed by an agency supervisor, and submitted to AAU via fax to 405-230-8017, which is also listed on the cover sheet.
It is important that you use the attached fax cover sheet to ensure proper routing. The form is not available on the website at this time; however, it can be provided through Provider Questions.
