The AAU has begun the process of converting all of our forms to meet the OKDHS guidelines for ADA Compliance. While this is a slow process, we are pleased to announce that several of our forms are now available on the OKDHS internet. You may access them by going to http://www.okdhs.org and following these steps:
- Click on "A-Z List of Programs/Services" under the "Quick Links" box
- Click "ADvantage Administration"
Note: Under the "Links for Case Managers" section, there are links to both the new OKDHS Case Management forms and the former version of the forms.
At this point, with the exception of the Service Plan Cost Sheet and Addendum, we have not set an official implementation date for use of the OKDHS forms; however, we encourage you to become familiar with the new forms and begin using them at your earliest convenience.
There are two important aspects to note about the new forms. First, all forms will be available in both PDF and Word format, which will allow you the ability to save completed forms to your computer. Second, all forms have been given a new number, which complies with OKDHS standards. All ADvantage forms will begin with 02CB, followed by a three-digit number. Whenever possible, the three-digit number has remained similar to the former ADv form number (ex: ADv5, Service Team Release of Information, is now 02CB005). Additionally, for easy reference, the former ADv form number is listed on the form as well.
In the coming weeks, you will receive communications titled, “AAU Bulletins on New Forms”. Each communication will provide detailed information for a specific form or set of forms along with some helpful tips to make transition to the new forms easier for your agency.
