CD-PASS Member Training
NEW: Consumer-Directed Personal Assistance Services and Supports (CD-PASS) service option is offering training to all active and prospective CD-PASS Members and/or their Authorized Representatives residing in the following eligible counties:
Adair, Alfalfa, Atoka, Beaver, Beckham, Blaine, Bryan, Canadian, Cherokee, Choctaw, Cimarron, Cleveland, Coal, Craig, Creek, Custer, Delaware, Dewey, Ellis, Garfield, Grant, Harper, Haskell, Hughes, Kay, Kingfisher, Latimer, Leflore, Logan, Major, Mayes, McCurtain, McIntosh, Muskogee, Noble, Nowata, Okfuskee, Okmulgee, Oklahoma, Osage, Ottawa, Pawnee, Payne, Pittsburg, Pontotoc, Pushmataha, Roger Mills, Rogers, Seminole, Sequoyah, Texas, Tulsa, Wagoner, Washington, Washita, Woods, and Woodward (See "CD-PASS Expansion" bulletin issued on 3/10/15 for more information).
Invitations: Current CD-PASS Members will receive an invitation by mail. Case Managers may also provide this notice to any prospective or current CD-PASS Members.
Training Dates: MSU-Tulsa will be hosting two, 3-hour training sessions for Members and/or their Authorized Representatives.
- 1p – 4p, Tues., April 8, 2015, MNTC-South Penn Campus, Main Building, Conference Rooms A&B
- 1p – 4p, Fri., April 9, 2015, OSU-Tulsa, North Hall, B.S. Roberts Room
Seating is limited, so only Members and their Authorized Representatives are encouraged to attend. If you have a Member on CD-PASS or interested in CD-PASS, please inform them of this training. To register, ADvantage Members should call our CareLine: 1-800-435-4711.
Maps of the Moore-Norman Tech Center (MNTC)-South Penn and OSU-Tulsa campuses are attached to this bulletin. Check-in on the day of each training will begin at 1:00PM.
