Step 1. Submit your comments or concerns by contacting the district office by telephone, letter or e-mail. You can find telephone numbers, mailing addresses, and e-mail addresses for district offices on the Child Support Offices page.
Step 2. If you fail to receive a response, or the response you receive is unsatisfactory, you may then contact the Center for Customer Services in writing at Oklahoma Child Support Services, Attn: Center for Customer Services , P.O. Box 25352, Oklahoma City, OK 73125-0352, by fax at (405) 522-3685, or e-mail.
To better provide our customers with a convenient way to raise concerns about services or treatment they receive, or request an administrative review, we offer Form 03EN012E, Child Support Comments (.pdf, 2 pp, 344 KB). You may download, print, fill out, and mail in this form. Please note that this process applies only to custodial persons, noncustodial parents, biological parents, or authorized representatives who have a case with the State of Oklahoma. If you have a case being handled by another state or a tribal office, please contact that state or tribal office.
Step 3. Please include the following information with your comments or concerns:
- First and last name
- Relationship to the case (i.e. mother, father, grandparent, etc.)
- Social Security number
- Case number/FGN (Family Group Number)
- Mailing address
- Telephone number
- Attorney of Record or authorized representative (if applicable)
- Detailed explanation of comment or concern
- Desired resolution
Step 4. The Center for Customer Services will send your comments to the district office for further review. Within 30 days of receipt of your comments, a review will be completed and any necessary action will be taken.
Step 5. If further review is requested, you may submit your comments to Center for Customer Services and the Oklahoma Child Support Services director will review them and take appropriate action within 30 days. You will be notified of the outcome of the review in writing. The result of this review is the final decision of OCSS.
