Dear Community Living Home Provider:

Per diem rates for Community Living Homes, 7 to 12 beds, were approved by the Oklahoma Health Care Authority and  are effective January 1, 2008.  The per diem rates for the procedure code T1020 at each level are as follows:  

   7 Beds -- $117.75
   8 Beds -- $108.00
   9 Beds -- $100.50
 10 Beds --   $94.50
 11 Beds --   $89.50
 12 Beds --   $84.50
