Dear Nutrition Services Provider:

RE:  Rate change

Effective July 1, 2006, the rate for Nutrition Services was reduced to $15.36 per quarter hour.  New service authorizations (“PB” numbers) will not be issued for DDSD waiver services for procedure codes 97802 and 97803.  Claims submitted for service provided on or after July 1, 2006 will pay at the new rate.  Please update your billing process to reflect the rate change.   

For state funded Nutrition services (procedure codes 97802 and 97803), new service authorizations (“MC” numbers) will be issued at the new rate for services.  Providers will be sent the new authorizations as soon as they are issued.
