Dear Vocational/Employment Provider:

The Monthly Attendance Record form (06WP044E), which was previously known as the DDS-44 and utilized for state funded employment (sheltered) services, has been revised effective January 20, 2007. The form will be piloted for 6 months before being finalized. We are requesting feedback from you regarding ways to make the form efficient and user friendly.

There are many new features on this form. When entering the social security number on the first page, the number will automatically enter on the corresponding line on the second page. The days claimed column would include the number of full service days and prorated days, as well as therapeutic leave days. By entering the total, the form will multiply this number by $21.00 to figure the monthly cost. The form will also calculate the total cost.

For your convenience, the form is on the OKDHS website at http://www.OKDHS.org. You may search for the form using the above form number. The form is available in both Microsoft Word and Excel. Either version of this form is acceptable. To print the entire Excel document, click “print entire workbook”.
