Dear Vocational/Employment Services Provider:

The Employment Progress Report form (06WP066E) has been revised and is now available through the internet on the OKDHS.org Web site. The form is used to provide documentation to the case manager of the provision and outcome of services.

The level of average weekly job coach intervention in an individual placement was added to the report. The percentage should be calculated based on the number of hours the job coach is on site with the individual. Listing the name of each staff providing employment services and identifying if they have been trained per DDSD policy was also added.

As a reminder, monthly progress reports are due by the 10th of the month. All fields are required for the form to be complete. If an individual receives more than two service types with your agency, additional forms should be added to complete the information. Piece rate wages must be converted to an hourly rate.
