Dear Assistive Technology Provider:
Your Developmental Disabilities Services Division (DDSD) waiver contract with the Oklahoma Health Care Authority (OHCA) will expire on October 31, 2006. DDSD will no longer require you to have an assistive technology contract and no renewal contracts will be offered. Instead, all existing and future DDSD authorizations to provide assistive technology will be issued against the Oklahoma Health Care Authority’s (OHCA) “DME & Medical Supplies” contract. In order to avoid a possible delay in payment, please follow the steps below to obtain an OHCA “DME & Medical Supplies” contract.
- Access the OHCA website at http://www.okhca.org/ (Link opens in new Window)
- Go to Providers and select Enrollment.
- On the Enrollment page, select New Contracts
- On the “Fee-For-Services Provider Contracts” page, select “DME & Medical Supplies”
- Complete the contract and all required attachments. Attach a copy of this letter.
- Remember to keep a copy of the contract for your files
- After OHCA receives and approves the contract, your existing provider ID# (formerly used for the DDSD waiver contract) will be updated with a contract end date of December 31, 2007.
