Dear Agency Companion Services Provider:
RE: Rate increase
Effective November 1, 2007, the following changes will occur in the Agency Companion rates.
Agencies that employ the Companion the rate will increase to:
AGENCY COMPANION (Employee) – ENHANCED: S5126 (TV) $147.00
AGENCY COMPANION (Employee) – CLOSE: S5126 TF (TV) $112.50
AGENCY COMPANION (Employee) – INTERMITTENT: S5126 U1 (TV) $78.00
Agencies that contract with the Companion the following rates have been developed:
AGENCY COMPANION (Contractor) – ENHANCED: S5126 TG (TV) $121.75
AGENCY COMPANION (Contractor) – CLOSE: S5126 U4 (TV) $93.50
New service authorizations (“PB” numbers) will not be issued for DDSD waiver services for the Employee procedure codes. Please be advised that dates of service on claims should not span November 1, 2007. Instead, claims for dates of service through October 31, 2007 should end on October 31, 2007 and new claims should be submitted for dates of service on or after November 1, 2007 in order for the appropriate rate to be paid. Any billings submitted for services at the previous rate on or after the effective date of the increase will be paid at the lower rate.
New service authorizations (“PB” numbers) will be issued for DDSD waiver services for the Contractor procedure codes. Providers should receive the new authorizations prior to November 1. Please update your billing process to reflect the rate for services provided on or after November 1, 2007.
