Dear Provider,

The enclosed training curriculum regarding incident reporting was developed to assist your agency in documenting reportable incidents on form 06MPO46E. Use of the training curriculum is not mandatory.

The curriculum identifies common reporting errors and provides examples of incident narratives. The curriculum may be used to meet annual training requirements and as in-service training for new staff. Staff may receive up to two hours of credit upon completion of the training.

An electronic copy of the curriculum, which can be easily printed and copied, is also being sent to your agency. If you do not receive the electronic copy by January 31, 2008, please contact Kim Akins at 405-521-4981 or kim.akins@OKDHS.org.
