Disclosure of Information and Special Power of Attorney (Authorized Representative)

Child Support case files and information are confidential

The OKDHS Child Support Enforcement Division (CSED) follows federal and state law and policy and only releases specific case information to authorized persons for authorized purposes. This includes requests for information over the phone, in writing, or in person. Unless your case information must be released to the other parent through a court action, CSED will only release your specific case information to you, your attorney of record, or your authorized representative. This confidentiality policy is governed by federal law found at: Title 42 United States Code §§ 453, 454, and 454A, 45 Code of Federal Regulations §§ 307.13; and state law found at: Title 43 Oklahoma Statutes § 112A (Link opens in new window); and Title 56 Oklahoma Statutes §§ 183 and 237(H) (Link opens in new window).

If you want to designate an authorized representative, who is not your attorney of record, to have access to your child support case information, you must use this Special Power of Attorney (Form: 03EN010E) (.pdf, 2 pp, 163 KB). We are providing below the text of state policy found at Oklahoma Administrative Code 340:25-1-3.1, effective July 1, 2002, which explains how you use the form.

The Special Power of Attorney form informs CSED:

  • who you authorize us to release your confidential information to;
  • the extent of access to records your authorized representative may be given; and
  • the actions your authorized representative may take on your behalf.

You will need to fill out and sign this form in front of a witness. To avoid even the appearance of impropriety or undue influence, you will not be able to have it witnessed at your district CSED office, but you will need to turn it in there.
