The Order/Notice to Withhold Income for Child Support form is a Federal standardized income withholding form, as required by Section 324 of the Personal Responsibility and Work Opportunity Reconciliation Act (PRWORA) of 1996 (P.O.04-93). It is available for use by tribunals/courts, individuals, and private agencies that need to complete and submit the Order/Notice to employers (and other debtors) for wage withholding.
