Insulet Corporation Issues Voluntary Recall of OmniPod Insulin Management System
On July 8, 2015, Insulet Corporation (Insulet or the Company) initiated a lot-specific voluntary recall of 40,846 boxes (10 Pods per box) of the OmniPod (Pod) Insulin Management System. This field corrective action is due to the possibility that some of the Pods from these lots may have a higher rate of failure than Insulet's current manufacturing standards.

The One Minute Miracle Inc. Issues Voluntary Nationwide Recall of MIRACLE DIET 30 Due to Presence of Undeclared Phenolphthalein & MIRACLE ROCK 48 Due to Presence of Undeclared Thiosildenafil
The One Minute Miracle Inc. is voluntarily recalling one lot each of Miracle Diet 30, capsules and Miracle Rock 48. Miracle Diet 30 has been found to contain undeclared phenolphthalein, phenolphthalein was an ingredient used in over-the counter laxatives but was removed from the market because of concerns of carcinogenicity.

General Mills Voluntarily Recalls a Limited Quantity of Frozen Cascadian Farm Cut Green Beans
General Mills today announced a voluntary Class I recall of a limited quantity of frozen Cascadian Farm Cut Green Beans produced over two days in March 2014. The recall is being issued as a precaution after one package of finished product tested positive for the presence of Listeria monocytogenes.

Sam Mills USA LLC is voluntarily recalling approximately 11,083 cases of - 4.4 ounce boxes of Gluten Free Chocolate Chip Granola Bars because of a risk of cross contamination with Dairy. The recall is being initiated because the Sam Mills Gluten Free Chocolate Chip Granola Bar packaging states that this is a Dairy free product and in fact it should be labeled that in “May contain Traces of Dairy”.

Jo’s Candies of Torrance, California is voluntarily recalling Trader Joe’s Dark Chocolate covered Honey Grahams with Sea Salt (SKU 55024), because it may contain milk, which was not declared on the label. People who have an allergy or severe sensitivity to milk run the risk of a serious or life-threatening allergic reaction if they consume these products. Two reactions have been reported to date.

No other products are impacted by this recall. This recall is being conducted in coordination with the U.S. Food and Drug Administration (FDA). Blue Square Market Issues Update to Voluntary Nationwide Recall of Kaboom Action Strips and Lida Daidaihua Due to the Presence of Undeclared Sulfoaildenafil, Sibutramine and Phenolphthalein
Blue Square Market Inc. of North Huntingdon, PA, is recalling "Kaboom Actions Strip 12 Pack" and LiDa DaiDaiHua to the user level after FDA analysis revealed the products contain undeclared active pharmaceutical ingredients. Kaboom was found to contain sulfoaildenafil and LiDa DaiDaiHua was found to contain sibutramine and phenolphthalein. Sibutramine is an appetite suppressant that was withdrawn from the U.S. market in October 2010 due to increased risk of heart attacks and strokes.

Angry Orchard Cider Company Announces Voluntary Recall of Select Cases of Angry Orchard Crisp Apple Hard Cider
Angry Orchard Cider Company, LLC, Cincinnati, Ohio, today announced that it is conducting a voluntary recall of select cases (24/12oz bottles), 12-packs and 6-packs of Angry Orchard Crisp Apple Hard Cider. The precautionary action comes after the Company received inquiries from consumers that had experienced broken bottles or bottles overflowing when opened. After conducting follow-up quality testing, the Company determined that the affected cider is limited to two batches, produced on June 15 and June 29, 2015.

The products include specific versions and lot numbers of the Torcon NB® Advantage Beacon® Tip Catheters (Catalog Prefix HNBR5.0), Royal Flush® Plus Beacon® Tip High-Flow Catheters (Catalog Prefix HNR4.0), and Slip-Cath® Beacon® Tip Catheters (Catalog Prefix SCBR5.0).
