File contains documentation pertaining to the case of Homeward Bound v. The Hissom Memorial Center used for auditing services received against the services billed. Includes affidavits of service, name of case manager or supervisor, client plan of care, and correspondence to or from the Oklahoma Health Care Authority.

Disposition:
Retain in office three (3) years, then transfer to the State Records Center. Retain in the State Records Center and destroy seven (7) years after exhaustion of all legal remedies provided all audits have been completed and all applicable audit reports have been accepted and resolved by all applicable federal and state agencies.
