Description:
Individual Habilitation Plan Committee working papers used to develop procedures for writing Individual Habilitation Plans. Includes correspondence, drafts, and Case Manager and Consumer requirements. Information is related to an ongoing lawsuit, the Hissom Memorial Center vs. Homeward Bound. The finalized Individual Habilitation Plan for each client is filed in the individual’s client case file.

Disposition:
Retain in office until no longer needed for administrative purposes, then transfer to the State Records Center. Retain in the State Records Center and destroy seven (7) years after exhaustion of all legal remedies, provided all audits have been completed and all applicable audit reports have been accepted and resolved by all applicable federal and state agencies.
