Specialized Services Nursing Home Contracts

| Description: |
Frequently referred to as OBRA (Omnibus Budget Reconciliation Act) Contract, Nursing Home Contract, and/or Specialized Service Contracts.The contract authorizes the payment rate to nursing homes. The file contains name, address and administrator of the nursing home and is executed by the Division Administrator for DDSD. The Oklahoma Health Care Authority is the state office of record, Schedule 95-06, (Series 6-10), Provider Enrollment Contracts (Fee for Service).

two (2) cubic feet per year.

| Disposition: |
Retain in office two (2) years after expiration date then transfer to OKDHS Records Center or the State Record Center. Retain in OKDHS Records Center or the State Records Center and destroy individual contracts seven (7) years after expiration provided all audits have been completed and all applicable audit reports have been accepted and resolved by all applicable federal and state agencies and no legal actions are pending. If legal action is pending, destroy two (2) years after exhaustion of all legal remedies provided: records meet all stipulated retention requirements.
