Files pertain to the licensing of child placing agencies, child care institutions, and emergency shelters. File contain applications for license; requests for license renewals; information sheets listing data about staff (e.g., educational backgrounds); T.B. test results and other medical information about day care home staff; copies of any applicable articles of incorporation; copies of facility health and fire safety inspection reports; visitation reports compiled by OKDHS staff; and correspondence.

Retain in office and destroy outdated staff information sheets, civil rights compliance forms, T.B. tests, medical forms, and compliance reviews annually provided updates to them have been received. Transfer individual files to Series 1-9 when applicable facility fails to renew its license or to Series 1-10 for facilities whose licenses are revoked or rejected by OKDHS.
