Description: Photographic identification badges issued to OKDHS employees and other authorized persons needing access to OKDHS facilities. Required for access to certain work areas, storage areas, or other areas where access is restricted.
Volume: Less than 1 cubic feet per year 
Disposition: Issue to authorized OKDHS employees and non-OKDHS employees as needed.
