1-2 Medicaid and Food Stamp Quality Control Case Records   Confidential Record (USDA Food and Nutrition Services Quality Control Manual 4125B; Health and Human Services HCFA Quality Control Manual; and CFR 275.12(c)(1))  (Record Copy)
Description: Files pertain to federally mandated audits of Medicaid and Food Stamp recipients. Audits are of cases selected at random by applicable federal agencies. Documents include face sheets, work sheets, financial data, field investigation notes, bank releases, and correspondence.
Volume: 100 cubic feet per year
Disposition: Retain in office two (2) years then transfer to the OKDHS Records Center or the State Records Center. Destroy records when they become five (5) years old, provided all audits have been completed and all applicable audit reports have been accepted and resolved by all applicable federal and state agencies and provided no legal actions are pending. If legal action is pending destroy two (2) years after exhaustion of all legal remedies provided records meet all stipulated retention requirements.
