File contains reference copies of OBRA Contract, Nursing Home Contract, and/or Specialized Service Contracts usually located in Developmental Disabilities Services Division (DDSD) area offices. The contract authorizes the payment rate to nursing homes. The file contains name, address and administrator of the nursing home and is executed by the Division Administrator for DDSD. The Developmental Disabilities Services Division/Community Services Unit is the agency’s office of records, Schedule 87-04, Series 33-3, Specialized Services Nursing Home Contracts.
