Description: Forms signed by clients declining to complete voter registration applications while applying for assistance at OKDHS County Offices, provided to clients as required by 42 USC §1973, and retained in accordance with [26 O.S., §4-109.2(D)].
Volume: 25 cubic feet per year
Disposition:

Retain in County Office two (2) years, then destroy.

OR

For records transferred to the OKDHS Records Center, transfer records to the State Records Center. Destroy records when they become two (2) years old.
