Files contain FAX Coversheet (OKDHS Form Adm-107, or equivalent) used to transmit information electronically over the telephone lines. Form contains date, time, number of pages, name and telephone numbers of person sending and receiving the faxed information.

File with applicable record transmitted and destroy when all stipulated retention periods for that record have been met, and permission to destroy records has been received from the State Records Administrator.
