15-1 Administrative Claims History  (Record Copy)
Description: Magnetic tapes containing claims processing data relative to travel expenditure, paid miscellaneous claims, client service claims, travel reimbursement, purchase orders, paid physician claims, clothing purchases and payments, utility payments, and telephone claims payments.
Disposition: Retain in office and (1) update as needed and (2) store one (1) set of backup tapes with the Department's Management Information Division. Destroy all tapes seven (7) years after last entry provided all audits have been completed and all applicable audit reports have been accepted and resolved for at least three (3) years and no legal actions are pending. If legal action is pending destroy tapes two (2) years after exhaustion of all legal remedies provided records meet all stipulated retention requirements.
