Library: Records Disposition Schedules OKDHS County Offices Consolidated Records Disposition Schedule 82-37 January 26, 2006 Client Records 2-7 Report of Food Stamp Over Issuance   Description: Form FSP-10 used to notify the Overpayment Unit of the State Office of food stamps received by ineligible individuals. Disposition: Transfer original and one (1) copy to the Overpayment Unit.  Retain one (1) copy in client case file and one (1) copy in the county office.  When the Overpayment Unit returns a completed copy, destroy the copy in the client case file and replace it with the completed copy. Retain the county copy until no longer required for administrative purposes, then destroy.
