2-2 Control Cards 
Description: Form ABCDM-73, control cards used to audit claims submitted by "providers who have given non-technical medical care" to "needy recipients" approved for such care. Information includes client names, case numbers, provider names and addresses, and amounts of payment approved.
Disposition: After final claim has been filed, transfer individual control card to cable case file.  If a patient transfers to another county, transfer control card to that county after provider has filed "final claim" in that county.
