Library: Records Disposition Schedules OKDHS County Offices Consolidated Records Disposition Schedule 82-37 January 26, 2006 Administrative Records 1-18 Home Maintenance Aid Profile and Record of Training (Form HMA-1)   Description: Form used to provide basic information to individuals wishing to provide home maintenance services to eligible clients. The form also records the training and experience of individuals who provide home maintenance services. Disposition: Retain in office five (5) years after person stops providing service, then destroy provided no legal actions are pending. If legal action is pending destroy two (2) years after exhaustion of all legal remedies provided records meet all stipulated retention requirements.
