340:1-11-46. Role of the immediate supervisor

Employee requests for reasonable accommodation are promptly addressed at the lowest possible level of supervision, as well as in the most cost-efficient and least disruptive manner possible. If a verbal request is made, the immediate supervisor will complete Form ADA-1, (.doc, 1 pp, 668 KB), (new Form 14CR003E). Upon receipt of a request, verbal or written, the immediate supervisor reviews it and refers it to the local coordinator for resolution. The immediate supervisor assists in the gathering of additional information, if needed, and confers with the local coordinator and local administrator in resolving the request at the local level.
