The Department will replace or make reimbursement for the eyeglasses or contact lens of any employee at Northern Oklahoma Resource Center (NORC), Southern Oklahoma Resource Center (SORC), Hissom Memorial Center, the Oklahoma School for the Deaf, the Oklahoma School for the Blind, and any juvenile institution operated by the Department, if eyeglasses or contact lens are damaged by a resident of the above schools, center or institutions while the employee is engaged in the performance of duties. The eyeglasses or contact lens replaced shall be of comparable kind, quality and cost; however, the Department is not authorized to make payment or reimbursement for eye examinations necessary for the replacement of, or reimbursement for, the eyeglasses or contact lens damaged while the employee is engaged in the performance of his/her duties. The employee will submit Appendix J of this Chapter denoting specific authority for the claim (HB 1542-1984 Legislature) to the person administratively responsible for the facility to process for payment or reimbursement with a copy of the invoice and employee accident report attached.
