Below is a list of official forms for the OKDHS Office for Civil Rights.  Download and print the form(s) that apply, and return by mail to the OKDHS Office for Civil Rights.

  • Discrimination Complaint - Employee (Form 14CR019E) (.pdf, 1 pp, 131 KB)
    Formerly known as the P-19 form, this document is to only be used by employees of OKDHS to file a complaint of discrimination. This form must be completed if you believe that you have been discriminated against under Title VII of the Civil Rights Act of 1964, as amended. This would include discrimination because of race, color, age, gender, national origin or religious belief, as well as sexual harassment.
  • Discrimination Complaint Form - Client or Vendor (Form 14CR001E) (.pdf, 1 pp, 150 KB)
    Formerly known as the OCR-1, this document is to be used by clients or vendors of OKDHS to file a complaint of discrimination. This form must be completed if you believe that you have been discriminated against under Title VI of the Civil Rights Act of 1964, as amended. This would include discrimination because of race, color, age, gender, national origin or religious belief, as well as sexual harassment.
  • Request for Reasonable Accommodation - Employee of Applicant for Employment (Form 14CR003E) )(.doc, 1 pp, 668 KB)     Formerly known as the ADA-1 form, this document is to only be used by employees of OKDHS to file a request for a reasonable accommodation. This form must be completed to make a request under the Americans with Disabilities Act of 1990.
