Below is a list of official forms for the OKDHS Oklahoma Child Care Services Some of the forms are not yet available online. You may contact the Oklahoma Child Care Services for a copy of the form.

Request for Home Star Certification (Form 07LC027E) (.pdf, 7 pp, 582 KB)
Formerly known as OCC-27, this is the official application for family child care homes to participate in the Reaching for the STARS program at the 1+, 2, and 3 STAR levels.

Request for Child Care Center Star Certification (Form 07LC025E) (.doc, 7 pp, 193 KB)
Formerly OCC-25, this is the official application for child care centers to participate in the Reaching for the STARS program at the 1+, 2 and 3 STAR levels.

Stars Request for Extension (Form 07LC074E) (.pdf, 1 pp, 24 KB)
Formerly OCC-74, this is the request for an extension for time to comply for participation in the Reaching for the STARS program.
