- This document is used for children born in the state of Oklahoma to establish paternity. Parents complete this form and send it to the Oklahoma State Department of Health, Division of Vital Records, to legally establish paternity for a child born out of wedlock. - This document is used to add the natural father's name to an adult child's birth certificate. It is required when parents of a child age 18 years or older complete form 03PA209E, Acknowledgment of Paternity. Completion of this document by the adult child gives consent to add the natural father's name, listed on form 03PA209E, to the adult child's birth certificate. - This document is used by OKDHS customers of the Child Support Enforcement Division to record money received or not received from the noncustodial parent for the support of the child(ren) named on the form. It is a sworn statement signed by the custodial person under the penalty of perjury. Complete, print and return to the county OKDHS office. - This legal document is used to calculate the child support obligation for a noncustodial parent (obligor). For the parents’ combined base monthly obligation amount needed for Line 6, refer to the Child Support Guideline Schedule in Section 119 of Title 43 (Link Opens In New Window) of the Oklahoma Statutes. - This document is used to inform parents of the availability of free Child Support Enforcement Division (CSED) services, who is eligible to receive those services, and what is necessary for parents to provide CSED in order to get child support services. The document describes the different kinds of child support services available and the cooperation CSED must have from applicants in order to provide child support services. - This document is used in conjunction with the Acknowledgment of Paternity form when the mother is married to a man not the biological father of the child at the time of birth or within 300 days before the birth. - This document is used by OKDHS customers of the Child Support Enforcement Division who believe they or their child(ren) could be at risk of emotional or physical harm if their home address is known. - Initial Notice to Employers (Form 03EN006E) (.pdf, 4 pp, 369 KB)
- This form is used by employers for any new employee or existing employee with an Oklahoma child support order. Oklahoma law requires the Oklahoma Centralized Support Registry to centrally process child support payments. Under Section 413 of Title 43 of Oklahoma Statutes, employers are required to provide the Centralized Support Registry with the information necessary to build child support accounts in order to distribute child support income assignment payments to custodial persons. Steps for the employer to follow to prepare new accounts for child support payments are included with the form. The form also serves as the official notice for the employer to comply with state law. - This document is a fact sheet about genetic testing, which is done to assist in establishing the paternity of a child. It consists of frequently asked questions about genetic testing and their answers. - Motion to Modify Child Support (Form 03EN013E) (.pdf, 3 pp, 255 KB)
- This legal document is used to request a court to modify a child support order. A Motion to Modify may be submitted when there has been a substantial and material change in circumstances to the child support order, such as: (1) A child for whom the order was entered has turned 18 and is no longer regularly enrolled and attending high school; (2) The father's income has changed; (3) The mother's income has changed; (4) The living arrangements of the child(ren) have changed; (5) The child care expenses have changed; or (6) There has been a change in health insurance or it was not addressed in a previous order. - This document is used to withdraw a sworn statement denying paternity of a child. When this document is completed and filed with the Oklahoma State Department of Health (OSDH), Division of Vital Records, the man named as the natural father on Form 03PA209E, Acknowledgment of Paternity, will no longer be the legal father and his name is removed from the child's birth certificate. The child's last name is then changed to the mother's last name at the time of the child's birth, and the man filing this document with the OSDH gets his name added to the child's birth certificate. - This document is used when the OKDHS Child Support Enforcement Division is NOT providing child support services, after a judge enters a child support order, Oklahoma law requires custodial persons, noncustodial parents or their attorneys complete this form and mail it to Oklahoma's Central Case Registry. This ensures that child support orders are properly recorded so that Oklahoma's Centralized Support Registry can distribute child support income assignment payments to the correct custodial person.
