Outcomes Report: Logic Models
Establishing Critical Linkages-Logic Models

To answer “What are our outcomes?” we must first establish links between what we do and the actual changes that take place in the lives of our customers.

These critical links help us to determine if the intended and actual results are similar.

A logic model is a top-level depiction of the flow of materials and processes to produce the results desired by an organization or program. It is a representation of the linkages between program activities and the changes those activities will produce.

The next page contains a visual representation of OCSS initial, intermediate and long term outcomes and linked goals.
