Office Information
The Center for Customer Services assists customers statewide by providing child support verification for housing assistance.
Please mail or fax a letter to the Center for Customer Service which contains the following information:
- Consent for Child Support Enforcement Division (CSED) to release child support information for housing purposes;
- Your social security number;
- A copy of your driver’s license or state ID to verify identity;
- The housing representative contact person;
- Information on where to send completed forms (mailing address or fax number); and
- Your signature.
