Name-Based Background Checks
 

Child welfare staff can obtain a name-based search by faxing a completed and signed 04AD003E (ADM-130) form to Gay Jones, (405) 522-3840. Please allow 3 to 5 working days for results.

After-hours (weekends, holidays or after 4:30 p.m. on weekdays) kinship background checks can be obtained through local law enforcement for a potential kinship resource of a child in protective custody.  (OKDHS Policy OAC 340:75-7-15(g))
