The Oklahoma Department of Human Services (OKDHS) and Tribal staff can obtain fingerprint-based background checks by submitting a completed Request for Background Check (form 04AD003E, formerly ADM-130) and 2 completed OKDHS issued fingerprint cards per person to the:

There is no cost to the OKDHS or Tribal applicant.

Fingerprinting Authorization Forms
This is an automated Finance Form. An approved county staff member can obtain the form. OKDHS will pay for required fingerprinting of OKDHS foster and adoptive applicants by local law enforcement or private fingerprinting sources. Tribal homes serving OKDHS custody children are also included in this process.

If you actively fingerprint for your county, the following supplies can be ordered by e-mailing Debbie King. Most orders will be sent out the same day. (We do not provide supplies to private agencies or attorneys.)
Assistive Private Adoption Applicants or Attorneys
Private adoptive applicants and attorneys may contact the Fingerprint Processing Section at (405) 522-3784 for assistance. A packet appropriate for private adoptions will be sent.
Fingerprint Services to the Foster Home and Adoptive Applicants
