What form(s) do I need?

For complaints about discrimination, there are two forms. One form, the Employee Discrimination Complaint Form (OKDHS Form 14CR019E)  (.doc, 1 pp, 929 KB) is used by OKDHS employees to file a complaint of discrimination. The other form, the Client or Vendor Discrimination Complaint Form (OKDHS Form 14CR001E), is used by clients or vendors doing business with OKDHS and who are filing a complaint of discrimination.

For filing a request for Reasonable Accommodation, there are two official OKDHS forms available. The first, the Request for Reasonable Accommodation (OKDHS Form 14CR003E), is used by OKDHS employees filing a request for Reasonable Accomodation. The second, the Request for Reasonable Accommodation (OKDHS Form 14CR004E ), is used by clients or vendors doing business with OKDHS and who are filing a request for a Reasonable Accomodation.
