  • Discrimination Complaint - Employee (Form 14CR019E) 
    Formerly known as the P-19 form, this document is to be used only by employees of DHS or applicants for employment with DHS to file a complaint of discrimination based on race, color, sex, religion, national origin, age, disability or genetic information.
  • Discrimination Complaint Form - Client or Vendor (Form 14CR001E) 
    Formerly known as the OCR-1, this document is to be used by clients, applicants for DHS services, or vendors of DHS to file a complaint of discrimination based on race, color, sex, religion, national origin, age, disability, genetic information, or political opinion or affiliation.
  • Request for Reasonable Accommodation (Form 14CR003E)
    Formerly known as the ADA-1 form and the ADA-2 form, this document is to be used by employees of DHS, applicants for employment with DHS, clients, applicants for DHS services, and vendors of DHS to file a request for a reasonable accommodation under the Americans with Disabilities Act Amendments Act.  Print, sign and return original to: Office for Civil Rights, P.
