    • This document is used for children born in the state of Oklahoma to establish paternity. Parents complete this form and send it to the Oklahoma State Department of Health, Division of Vital Records, to legally establish paternity for a child born out of wedlock. Under Oklahoma law, completion of this form has the same legal effect of establishing paternity as a court order. Complete, print and return to the address located on the last page of the form.     • This document is used to add the natural father's name to an adult child's birth certificate. It is required when parents of a child age 18 years or older complete form 03PA209E, Acknowledgment of Paternity. Completion of this document by the adult child gives consent to add the natural father's name, listed on form 03PA209E, to the adult child's birth certificate.   • Affidavit of Child Support Payments Received (Form: 03EN002E) (.pdf, 3 pp, 249 KB)
    • This document is used by OKDHS customers of Oklahoma Child Support Services to record money received or not received from the noncustodial parent for the support of the child(ren) named on the form. It is a sworn statement signed by the custodial person under the penalty of perjury. Complete, print and return to the county OKDHS office.   • Application for Child Support Services (Form 03EN001E) (.pdf, 13 pp, 761 KB) 
    • A custodial person or noncustodial parent who needs child support services for a minor child uses this application to apply.   • Child Support Comments (Form 03EN012E) (.pdf, 2 pp, 344 KB)
    • A customer who wants to comment upon OCSS services or express other concerns uses this form.   • Child Support Computation (Form 03EN025E) (.pdf, 6 pp, 433 KB)
    • The Child Support Computation Form is the legal document used to calculate the child support obligation for a noncustodial parent (obligor) due on or after July 1, 2009 as required pursuant to Section 120 of Title 43 (Link opens in new window) of the Oklahoma Statutes. Review the Instructions (.pdf, 23 pp, 393 KB) for information on how to complete this form and for the Insurance Premium Worksheet, the Oklahoma Schedule of Basic Child Support Obligations table, the OKDHS Child Care Eligibility/Rates Schedule C-4 and the Oklahoma Cash Medical Income Guidelines table. The form must be signed by the judge and attached as an exhibit to orders which establish or modify a child support obligation.   • Child Support Computation for Period Prior to July 1, 2009 (Form 03EN005E) (.pdf, 4 pp, 291 KB)
    • This legal document is used to calculate judgments in establishment cases applying the child support guidelines law for time periods through June 30, 2009. See Title 10 Section 83 (Link opens in new window) and Title 56 Section 238.6B (Link opens in new window). Review the Instructions (.pdf, 4 pp, 47 KB) for information on how to complete this form. For the parents' combined monthly obligation amount needed for Line 6, refer to the Child Support Guidelines Schedule in Section 119 of Title 43 (Link opens in new window) of the Oklahoma Statutes.   • Child Support Order (Standard) (Form 03ES010E) (pdf, 13 pp, 406 KB)
    • This legal document is the child support order used to establish a child support obligation for a child(ren) in a dissolution of marriage or paternity action.  Please print your completed form if you would like a copy for your records.   • Child Support Order (Child Welfare Cases) (Form 03EN015E) (pdf, 11 pp, 335 KB) 
    • This legal document is the child support order used to establish a child support obligation for a child(ren) in a deprived action.  This form is required under Section 1-4-702 of Title 10A (Link opens in new window) of the Oklahoma Statutes, effective 11/01/05 renumbered 5/21/09. Please print your completed form if you would like a copy for your records.   • Child Support Services and Responsibilities (Appendix 03AX016E) (.pdf, 4 pp, 86 KB)
    • This document is used to inform parents of the availability of free Oklahoma Child Support Services', who is eligible to receive those services, and what is necessary for parents to provide OCSS in order to get child support services. The document describes the different kinds of child support services available and the cooperation OCSS must have from applicants in order to provide child support services.     • This document is used in conjunction with the Acknowledgment of Paternity form when the mother is married to a man not the biological father of the child at the time of birth or within 300 days before the birth. An Acknowledgment of Paternity form, signed by the mother and the biological father, must be completed, signed and accompany the Denial of Paternity form when filed with the Oklahoma State Department of Health, Division of Vital Records. complete, print and return to the address located on the last page of the form.   • Designation of Custodial Person's Address of Record (Form 03EN026E)  (.pdf, 2 pp, 156 KB)
    • This form is used by a custodial person who wishes to establish his or her attorney as the address of record on the case. By using this form, the custodial person agrees that all information regarding the child support case will be mailed to the attorney rather than directly to the custodial person. Child support payments will also be issued as a paper check and mailed to the attorney’s address, rather than being direct deposited or issued electronically on a debit card. Both the custodial person and the attorney must sign this form in order for it to be effective. Additionally, both the custodial person and the attorney must sign the  Termination of Attorney Address of Record Designation (Form 03EN030E) (.pdf, 2 pp, 167 KB) in order to change the address of record and the way child support payments are issued.   • Family Violence - Address of Record Statement (Form 03EN008E) (.pdf, 2 pp, 249 KB)
    • This document is used by customers of Oklahoma Child Support Services who believe they or their child(ren) could be at risk of emotional or physical harm if their home address is known. Complete, print and return to the address located on the last page of the form.   • Initial Notice to Employers (Form 03EN006E) (.pdf, 4 pp, 369 KB)
    • This document is used by employers for any new employee or existing employee with an Oklahoma child support order.  Oklahoma law requires the Oklahoma Centralized Support Registry to centrally process child support payments. Under Section 413 of Title 43 of Oklahoma Statutes, employers are required to provide the Centralized Support Registry with the information necessary to build child support accounts in order to distribute child support income assignment payments to custodial persons. Steps for the employer to follow to prepare new accounts for child support payments are included with the form. The form also serves as the official notice for the employer to comply with state law.     • A customer who wants someone else to translate confidential case information discussed on their behalf uses this form to designate an interpreter.  Complete, print and return to the local Child Support Office. Please print your completed form if you would like a copy for your records.     • This document is a fact sheet about genetic testing, which is done to assist in establishing the paternity of a child. It consists of frequently asked questions about genetic testing and their answers.

  • Locate Only Rights and Responsibilities (Form 03EN007E) (.pdf, 1 pp, 125 KB)
    • This document is a companion to Form 03EN001E, Application for Child Support Services.  A custodial parent, legal guardian, attorney, or agent of a child may apply for locate-only services for the purpose of discovering location or employment information that will lead to establishing paternity, establishing, modifying, and enforcing child support obligations.  Please print this completed form and return it with your Application for Child Support Services.   • Motion to Modify Child Support in District Court - Instructions and Forms (Form 03EN017E) (.pdf, 28 pp, 608 KB)
    • This packet contains self-help instructions and forms to help parents represent themselves in child support modification actions. The forms are used to ask a district court to change a child support order and are required under Sections 118I and 120 of Title 43 (Link opens in new window) of the Oklahoma Statutes, effective 11/01/06.     • This packet contains self-help instructions and forms to help parents represent themselves in child support modification actions. The forms are used to ask the OKDHS Administrative Hearings Court to change a child support order and are required under Sections 118I and 120 of Title 43 (Link opens in new window) of the Oklahoma Statutes, effective 11/01/06.     • This document is used to notify employers and other withholders to deduct child support payments from noncustodial parents' income and to send the payments to Oklahoma's Centralized Support Registry for distribution to custodial persons.     • This document is used by Oklahoma Child Support Services to release an address of record for service of process in support, visitation or custody court actions.     • This document is used to rescind (cancel) the legal establishment of paternity created by a previously submitted acknowledgment of paternity. This form must be completed and filed with the Oklahoma State Department of Health, Division of Vital Records within 60 days of the acknowledgment of paternity.     • This document is used to withdraw a sworn statement denying paternity of a child. When this document is completed and filed with the Oklahoma State Department of Health (OSDH), Division of Vital Records, the man named as the natural father on Form 03PA209E, Acknowledgment of Paternity, will no longer be the legal father and his name is removed from the child's birth certificate. The child's last name is then changed to the mother's last name at the time of the child's birth, and the man filing this document with the OSDH gets his name added to the child's birth certificate.     • This document is used by Oklahoma Child Support Services to allow someone else to receive their confidential case information over the telephone or in writing.     • This document is used when Oklahoma Child Support Services is NOT providing child support services, after a judge enters a child support order, Oklahoma law requires custodial persons, noncustodial parents or their attorneys complete this form and mail it to Oklahoma's Central Case Registry. This ensures that child support orders are properly recorded so that Oklahoma's Centralized Support Registry can distribute child support income assignment payments to the correct custodial person.
