Protective Intervention Plans (PIP)

| Description: |
Files contain, but are not limited to, semi-annual psychotropic medication review forms, protective intervention plans, staff books, daily schedules, and annual review updates for DDSD consumers. This includes DSM IV diagnosis, serious risks or dangerous behaviors, risk assessments, plan responsibilities, education and training components, preventive supports, restrictive or intrusive behavioral procedures, and evaluations of plan effectiveness.

Retain in office and transfer to an electronic format. Destroy hard copy documents upon certification that all records have been successfully entered into an electronic format and after storing an electronic back up copy in an off site storage area and after obtaining approval from the State Records Administrator to destroy the hard copy documents.
Delete electronic and destroy all below standard records twenty-five (25) years after the death of the client or twenty-five (25) years after last contact with client, provided all audits have been completed, all applicable audit reports have been accepted and resolved by all applicable federal and state agencies, no legal actions are pending, and approval has been received from the State Records Administrator to destroy records. If legal action is pending, destroy two (2) years after exhaustion of all legal remedies, provided records meet all stipulated retention requirements. Added January 26, 2006
