(a) The State court provides a copy of the final decree of adoption of any Indian child to the Secretary of Interior (Secretary). Additional information which accompanies the final decree, if not included in the final decree, includes the:
- (1) name and tribal affiliation of the child;
- (2) names and addresses of the biological parents;
- (3) names and addresses of the adoptive parents; and
- (4) identity of any agency having files or information relating to such adoptive placement.
(b) If the biological parent or parents request that their identity remain confidential, the court includes the affidavit with the other information. The Secretary insures that confidentiality is maintained.
